Personal Information to Be Collected and Used:
Applicant’s name, address, contact information, user’s name, user’s age, gender, etc.

Retention and Use Period of Personal Information:
Until the consultation is completed

Third-Party Information Sharing Consent (Required)

CAREHUB may provide personal information to third parties as follows when necessary for consultation requests, admission counseling, medical and care coordination, emergency response, and service provision.

1. Recipients of Personal Information

  • Medical institutions and emergency medical service providers: hospitals, clinics, home-visit medical care providers, 119 emergency services, emergency rooms, etc.

  • Pharmacies

  • Relevant administrative authorities: local governments, public health centers, etc.

  • Guardians

  • External professional organizations and partner institutions: service coordination organizations, AI analysis organizations, equipment operation and technical verification companies, etc.

2. Purpose of Provision

  • Medical coordination, health checkups, home-visit medical care, and emergency response

  • Prescription, dispensing, and medication management

  • Reporting and administrative processing in accordance with applicable laws and regulations

  • Providing guardians with information on residents’ conditions and service-related matters

  • Service coordination, behavioral data analysis, technical verification, service model validation, test-bed operation, and PoC execution

3. Items Provided

  • Basic information: name, date of birth, gender, contact information, address

  • Guardian information: guardian’s name and contact information

  • Consultation and service information: consultation request details, service usage information, service provision records

  • Health and medical information: diseases, medication information, medical opinions, test results, etc.

  • Accident and emergency-related records

Sensitive information, including health and medical information, will be provided only to the minimum extent necessary to achieve the purpose of provision.

4. Retention and Use Period

Personal information will be retained and used until the purpose of provision is achieved or for the retention period required under applicable laws and regulations.

5. Right to Refuse Consent and Disadvantages

The data subject may refuse to consent to the provision of personal information to third parties. However, refusal to consent may limit the provision of certain services, such as consultation requests, medical and care coordination, and emergency response.

Marketing Consent (Optional)

Care Hub (CAREHUB) collects and uses personal information as follows to provide service updates and event information.

Purpose: Introduction of new locations and services, promotions, events, customer surveys and satisfaction research.

Data collected: Name, mobile phone number.

Retention: Until consent for receiving marketing information is withdrawn.

Marketing information may be sent via SMS, phone calls, or email.

This consent is optional, and you may use Care Hub (CAREHUB) services without agreeing.

You may withdraw your consent at any time through the Care Hub support team, and marketing communication will stop immediately upon withdrawal.

Withdrawal can be requested via the customer center (1551-5080) or in writing.
